If you attended a diocesan elementary or high school that is now closed, and would like to obtain a copy of your school transcript, please fill out the form attached here and mail it, along with a self-addressed stamped envelope to the Archives & Records Center: 1050 Logue Street, Pittsburgh, PA 15220.
