In order to obtain the sacramental information someone who is getting married, someone who will serve as a sponsor, or someone wishing to join your parish (or any other reason for you to request their sacramental information), please fill out the top portion of the chosen form on the left.
Once you fill out the entirety of the top portion of the form, you email it to the Archives & Records Center at [email protected]. Once we find the sacramental information, we will fill out the bottom portion of the document and email the completed document back to you.
Again, these forms are only for use by Diocese of Pittsburgh offices and parishes. Personal
records requests, and requests from outside of our diocese must use another form and process.

Q&A concerning Sacramental Record Requests:

1. Who can use these forms?
These forms can be used by diocesan parishes, schools or central administration offices that need verification from the Archives that an individual received one or more sacraments in one of our closed diocesan parishes. The verification process can be conducted via email.
2. Can these forms be used to request verification of a sacrament from other dioceses?
No, these forms can only be used within the Diocese of Pittsburgh.
3. Can we give this form to an individual and have that person send it to the Archives?
No, these forms cannot be used by individuals that do not work for a diocesan office, parish or school. We will not respond to any request sent in by anyone other than a diocesan office, school or parish.
4. Why are we making this change?
There are several reasons for making this change. The first is to streamline the process of verification. The current procedure has the parish tell an individual to get a certificate, having them contact the Archives, having the Archives send them the certificate and then having them give the certificate to the parish. Under the new procedure, the parish can contact the Archives directly and receive the information directly by us. Second, this can all be done by email, speeding up the process. Third, it will eliminate the need for multiple certificates to be created and stored.
5. What parish name do we use when the sacrament was received in a parish that subsequently merged to form a new parish?
The parish name entered should be the parish that existed when the sacrament was received. For example, if an individual was baptized at St. Adalbert Church in 1988, then the parish would be St. Adalbert. If the individual was baptized at the St. Adalbert Church building in 1994, then the parish would be listed as Prince of Peace.
6. Is this form an acceptable substitute for an actual certificate?Yes. If you have this form from the Archives, you do not need a separate copy of a certificate.
